CHASS is not a free clinic. CHASS services are billed at the time of visit. Co-pays and deductibles are calculated at the end of each visit. Payment in full is expected at the time the service is rendered. Minimum payment is $20.00. To determine applicable fee schedules please bring the following items to your first appointment:

Non-insured patients will be charged on a sliding-fee based upon household income and family size. Payment in full is expected at the time the service is rendered. Minimum payment is $20.00.
If you would like a routine visit or check-up, we will do our best to get you scheduled within 90 calendar days. If you are sick, we will do our best to schedule you the same day.

The Community Health and Social Services (CHASS) Center, Inc., is a community-based, not for profit organization formed to develop, promote, and provide comprehensive, accessible and affordable quality primary health care and support services to all residents of the community, with special emphasis on the underserved African-American and Latino population. CHASS is committed to the overall well-being of the community. CHASS provides comprehensive primary care services to people 18 years and older.
Pediatric and Adolescent Care
Our team of Family Medicine Providers offers competent, comprehensive and compassionate care to clients under 18 years. We follow well child care guidelines set by the American Academy of Pediatrics (AAP). Additionally, CHASS Providers care for the child in the context of the family, culture and community.
Services include: 1-2 day of age bilirubin check, all AAP recommended well child screenings, as well as comprehensive primary health care, and integrative specialty care with Henry Ford Hospital and Children’s Hospital of Michigan. Immunizations include all AAP recommended vaccines.
Prenatal visits to a CHASS provider include a physical exam, weight checks, blood pressure and laboratory screening, fetal assessment. Depending on the stage of the pregnancy, providers may also do blood tests and imaging tests, such as ultrasound exams. These visits also include discussions about the mother's health, the infant's health, breastfeeding education and advice, and any questions about the pregnancy.
First-time moms may be eligible to receive the services of a Doula through CHASS’ Marvelous Mom/Madre Maravillosa program. This program provides a Doula to women throughout their pregnancy and up to six months postpartum.
CHASS' Benefits Coordinators will also insure that moms are enrolled in all available programs including Medicaid and WIC (Women, Infant and Children Supplemental Nutrition) program.
If you are a senior, expectant mother or have children under the age of five (5), CHASS provides door-to-door transportation in certain areas. Please let the person scheduling your appointment know that transportation is needed.
Spanish service available. Open late on Tuesdays. Also offers nutrition and exercise classes.
